GROUP HEALTH INSURANCE PROGRAM

 

 

Following is information regarding the various insurance product lines we offer our employees. 

Firstly, a few points of interest, otherwise known as rules, pertaining to participation in the program:

 

 

 

 

Employees are eligible to participate in the health programs on the 1st of the month following hire date.  You have 30 days from your hire date in which to enroll or decline coverage(s).  After that time, you are ineligible to participate until the April 1 open enrollment period unless you experience a qualifying event, i.e., loss of current coverage, marriage, divorce, birth/adoption of child, etc.

 

 

 

Premiums are withheld on a pre-tax basis for medical, dental and vision coverage.  Premiums for all products are withheld on a weekly basis.

 

 

 

 

 

Premiums withheld pay for the next month's coverage, i.e., April premiums pay for May coverage.

 

 

 

 

 

 

You may participate in one, two, or all of the product lines.  The complete health program information and applications are sent to you along with your hire packet documents (it contains a few other individually required documents as well as the applications).

 

 

 

 

 

You must respond to Tech Plus regarding your choice of participation: 

 

TO ENROLL:  submit the applicable product line application(s);

TO DECLINE:  submit the single page "Benefit Election Recap" and check the "I decline coverage in the group health insurance program" box; or check the "I waive participation" box on your Work Assignment document.
